History
Mrs. A, a 35-year-old female patient of Russian nationality, was admitted with the above symptoms after being struck from behind by an electric bicycle while walking around 18:00 on June 26, 2025.
Mrs. A was traveling in Japan with her husband and had planned a four-day trip to briefly visit South Korea on their way back home. The accident occurred the very next day after starting her trip in Korea, while she was peacefully walking in downtown Seoul.
Initially, it was hard to comprehend how colliding with a bicycle could cause such severe injury. However, Mrs. A was completely unprepared, and the bicycle, which suddenly appeared from behind, was an electric bicycle. It collided with unexpected speed and force, causing the patient to lose her balance and fall forward violently, resulting in secondary injuries.

Neurosurgery found no neurological deficits, and X-ray readings showed a fractured right clavicle with displacement. The orthopedic surgeon performed clavicle fracture surgery under general anesthesia. Neurologically, the patient was at high risk for intracranial hemorrhage changes due to blood pressure fluctuations, necessitating vigilance for post-operative neurological changes and intracranial bleeding.
Results
The surgery was completed successfully. Because the patient’s clavicle was relatively thin, the post-operative rehabilitation process became very important. The wound will be disinfected every other day, and sutures can be removed on the second week. However, a Velpeau arm sling shoulder brace must be worn for at least 4 weeks.
After 4-6 weeks, daily activities like eating would be possible. Light exercise could begin while monitoring progress via X-rays, but full-fledged exercise might require over 6 months.
The attending physician stated, “While the fusion process at the fracture site must be monitored for clarity, a clavicle fracture alone is not expected to cause permanent loss of important bodily functions. If adequate orthopedic rehabilitation is performed, the likelihood of functional impairment appears low.”
